Tav said he's working on fixing the icons (see change log), and already fixed some of the other issues earlier today (missing icons in the lists in layers, objects and path effects dialog).
Thanks, didn't know about that list! You obviously did a much better job collecting detailed information than me.
You caught a few that I had missed earlier ;-)
* 'distribute-graph' used on the Connector tool controls bar and in the 'Align and Distribute' dialog
* 'text_rotation' on the text tool controls bar
Both have now been added to the list.
