I don't have a direct answer, but here are some ideas:
I have had issues when printing directly from Inkscape, so sometimes I save the file as a pdf, which will keep the actual size and then do the printing with another program.
Most of the problems come from the print dialog box that comes up on your computer. Look deeper into the options that are available there. Or try the PDF with other programs and see what kind of print options there are.
Inkscape will send actual size to the print spooler or whatever it's called. On a very regular basis I cut painting stencils for logos on gym floors that can be 12 feet high and 20 feet long. Pretty sure the issue is the printer, not Inkscape.
I use an older custom script which converts an exported EPS which then sends hpgl files to a 48" Graphtec cutter. Confusing, but it works for me. So yes, I think I have run into every problem possible over the past decade I've been doing this.

I'll place the real size sewing pattern on this template and print as A4 pages with regular printer.
Gluing them together.
Cutting by hand along the sewing patterns.
Put on the fabric and cut the fabric.
